Departing employee name: Please verify correct spelling and preferred name for account(s). The following information should be emailed directly to the Director of Technology for confidentiality purposes. Following information must be provided by the employee's supervisor.
Phone request checklist:
- Where should incoming calls to their phone go?
- What should be done with their voice-mail box?
- Last day of work or last day of access required for this employees resources & assets
Email request checklist:
- Do we disable employee's access to email account?
- Should the email account be deleted or forwarded? If so to whom and for how long?
- If the email account it kept does it need an auto reply? If so what should it say and for how long?
- Are their any messages, files, calendars, or shared mailing lists that need to be transferred to another employee?
- What is the room location?
- Do their files need to be transferred to another employee?
- Who goes through the old files?
- Do you need physical removal of computer during departure of employee?
Admin systems checklist:
- Disable Access to network resources: Filemaker databases, network folder shares, Moodle , Jenzabar etc.
